Our Services
Person-centered support for individuals with intellectual and/or developmental disabilities, provided through Oklahoma DHS Developmental Disabilities Services (DDS) waiver programs and serving communities across central and southwest Oklahoma.
Habilitation Training Specialist (HTS) Services
Our Habilitation Training Specialists are trained direct support staff, employed and supervised by Reach Out, who work with individuals with intellectual and/or developmental disabilities in the family home, in their own home, or alongside residential supports.
Working one-on-one, HTS staff help each person build and keep the skills that support a safe, healthy, connected life — always at a pace that respects each person's unique abilities.
- Daily living skills such as personal hygiene, meal preparation, and money management
- Medication administration
- Health and safety
- Social skills and community involvement
Daily Living Supports (DLS)
Daily Living Supports help adults (18 and older) with intellectual and/or developmental disabilities live as independently as possible in a shared home in the community, typically with two housemates.
Our trained staff, who do not live in the home, support each person with daily care and medical needs and create everyday opportunities to take part in community life.
Contact UsAgency Companion Services
Agency Companion Services offer adults (18 and older) a shared living arrangement in the home of a trained companion who is contracted with and supported by Reach Out.
The individual becomes part of the companion's household. The companion helps with daily care, health and medical needs, and daily living skills, and makes sure the person is included in community life.
Contact UsEmployment & Job Coaching
We help individuals find and keep meaningful work in their community. Our job coaches provide planned, hands-on training and support right at the job site, and work alongside employers to help each person succeed.
As a person gains confidence and independence on the job, coaching gradually steps back — and we stay in touch with ongoing follow-up support.
- One-on-one job coaching
- Group job coaching
- Ongoing follow-up support after coaching
Transportation Services
Getting from place to place can be one of the biggest barriers to independence in rural Oklahoma. Our transportation services provide safe, reliable rides so individuals can access medical appointments, community activities, employment, shopping, and more.
Our fleet of wheelchair-accessible vans and trained drivers helps ensure that distance is never a barrier to living a full, connected life.
